Throughout 2009 IAFP is offering educational opportunities around PCMH. We are in the midst of a joint educational collaboration with the Iowa Healthcare Collaborative and at our annual meeting this November 4 -6, 2009, we will offer PCMH topics for CME.
Below you will find multiple links to a number of great sources of PCMH information. A favorite of mine is the TransforMED website at
http://www.transformed.com. It offers a huge collection of useful, relevant information and tools for you to explore and use. Especially helpful is the Medical Home IQ (MHIQ) survey, which you can take to assess your practice’s strengths and weaknesses with regards to the PCMH.
